INSTRUCTIONS 
Welcome to our online store where you can view and choose photo products for the photo day.
  • You need one form per dance per child, i.e. if your child is in 5 dances, you need to complete 5 forms.
  • Forms can be completed either digitally on your computer or you may pick up hard copies from your dance school. 
  • If completed digitally, please print and bring to the photo day 2 copies and save the file for your records.
